Summary
Summary
Evaluates patient’s condition and/or disability, by selecting and administering appropriate tests and measures; formulates and administers a treatment plan; interprets testing and treatment to staff, patient, family; determines need for referral to other specialists and/or agencies; maintains proper records and reports.
EducationMust have a Master’s degree in Speech-Language Pathology.
Licensure/CertificationMust hold the Certificate of Clinical Competence (C.C.C.) from the American Speech-Language-Hearing Association (ASHA). Must hold or be eligible to receive a Speech-Language Pathology license in the State of Indiana, from the Indiana Professional Licensure Board. Must have a valid state driver's license, reliable transportation, and proof of insurance. Must have current CPR certification.

ExperienceAt least one year experience in a clinical setting related to the departmental scope of service provided. In lieu of previous work experience, verification of supervised clinical experience in evaluation and treatment of communication disorders.
Other QualificationsMust be willing to travel within the workday and be willing to make home visits. Must be able to lift 50 lbs.
