Summary
Summary
The Diabetes Care & Education Specialist RN (DCES) is responsible for inpatient and outpatient education for diabetic patients within the Standard of Care set by Parkview Health and meets the American Diabetes Association guidelines for accreditation. Delivers comprehensive and seamless care that integrates clinical and self-management aspects of diabetes and other comorbidities. Reviews medication for therapeutic effectiveness appropriate to patient needs and in accordance with evidence-based practice. Provides training and support relating to Insulin Pumps and Continuous Glucose Monitors. Utilizes clinical judgment and critical thinking skills in assessing and analyzing patient data as well as evaluating patient responses to therapy. Collaborates with patient to determine patient-centered goals in order to improve their health and to improve self-management of their diabetes. Is an integral part of the inter-professional team and provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life across the lifespan. Promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. Completes accurate and timely documentation in the EMR.
Education
Associate of Science in Nursing (ASN) required. Bachelor of Science in Nursing (BSN) required within 3 years of start date.
License
Associate of Science in Nursing (ASN) required. Bachelor of Science in Nursing (BSN) required within 3 years of start date.
Experience
5 years experience preferred working as an RN in clinical setting.
Other Qualifications
Demonstrates excellent customer service, establishes priorities, and works independently. Must have good verbal, written, and interpersonal communication skills.
