Lifestyle Change Specialist
Summary
Summary
Assesses, plans, and implements supporting lifestyle change principles and applications for assigned population, based on collaboration with clinical and non-clinical care team member prescriptions and recommendations. Assists in role specific administration and follow-up of care plan. Supports care team members to assure patient is leveraging best practices based on recommendations to improve patient-specific health profile. Coaches and motivates patients and families to attain best self through goal setting and accountability. Evaluates intervention for health outcomes and needed improvements. Stays abreast with current best practices and research related to effective principles of lifestyle and behavior change techniques and applications in various populations and care settings. Acts as resource to other staff and client companies on a variety of health related topics.
Education
Must have a Bachelor's degree in the area of Health Education, Exercise Physiology, Nursing, Dietary or a related field.
License/Certification
Must have a valid driver's license. Current CPR (AHA) certification required. Must have obtained NBHWC certification upon hire, or obtain certification within 1 year of hire date.
Experience
At least one to three years experience in wellness and health promotion preferred. Experience in a clinical setting, collaborating with a multi-disciplinary team is preferred.
Other Qualifications
Must have good presentation, verbal, and written communication skills. Must be able to lift 50 pounds; 100 pounds shared lifting.
