Premium Outpatient PT
Summary
Provides professional patient care and physical therapy services, including evaluation, planning, implementation, and documentation of treatment. Assesses and treats patients as per physician orders using various therapeutic procedures and assistive devices to maximize functional potential in all areas of the patient's life. Develops and implements individualized treatment plans with achievable functional goals and revises them as needed.
Collaborates with and provides appropriate supervision to Physical Therapist Assistants (PTAs) regarding treatment procedures. Educates patients, families, and caregivers on therapeutic exercises and home programs to continue following discharge. Ensures timely and accurate documentation and billing. All care is provided in alignment with the hospital’s philosophy, rehabilitation department standards, the Indiana State Practice Act, the American Physical Therapy Association, and The Joint Commission.
Education
- Must be a graduate of an accredited school of physical therapy with the following degree requirements:
- Before 1/1/2016: Minimum of a Bachelor’s degree
- After 1/1/2016: Doctorate of Physical Therapy (DPT) required
- International Candidates: Must hold a minimum of a Bachelor’s degree in Physical Therapy (DPT preferred)
Licensure/Certification
- Must hold or be eligible for a Physical Therapist license in the state where services are provided.
- State licensure must be obtained within 90 days of employment.
- CPR certification required within 90 days of employment.
- Must maintain all certifications and licensure per expiration dates.
- American Physical Therapy Association (APTA) membership preferred.
- If assigned to drive a company vehicle, a valid driver’s license is required.
- If assigned to drive a personal vehicle, proof of insurance is required in addition to a valid driver’s license.
Experience
- Work experience preferred but not required, beyond clinical experience obtained through an educational training program.
Other Qualifications
- Must be able to lift 50 pounds, assist with 100 pounds, and may be required to push up to 300 pounds.
- Strong verbal and written communication skills.
- Must be willing to work at other locations as assigned.
