PPG Manager Care Coordination
Summary
Summary
Through accountability to the PPG Director of Patient Care and Internal Resources, the PPG Manager Care Coordination leads and coordinates the activities of the PPG Care Coordination Teams including: Care Navigators/Coordinators, Social Support Specialists, and Medicare Wellness Nurses. Responsible and accountable for translating the mission, strategic goals and program priorities. Supports and maintains a culture of continuous process improvement as it relates to functions of the PPG Care Coordination Team.
Education
Must have a BSN from accredited school of nursing. Acceptable accrediting agencies are Commission on Collegiate Education in Nursing (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA). Master’s degree in healthcare related field or MSN preferred.
Licensure/Certification
Must be an RN with current Indiana licensure.
Experience
5 years of leadership experience. 5 years of nursing experience preferably in Ambulatory Care, Community Public Health, Case Management, or Care Coordination. Experience in project management and process improvement.
Other Qualifications
Strong foundation on leadership principles and application. Excellent written and verbal communication skills. Strong critical thinking skills. Ability to lead and direct co-works in a positive and professional manner. Expertise to facilitate change in population heath continuum. Demonstrated skill and/or experience in following: LEAN, root-cause analysis, problem solving methodologies and team dynamics.
