W/C Nurse Navigator Supervisor
Summary
Summary
Supervises the daily operation of assigned staff members to ensure that quality patient care is provided. Primary role is to assess patient/family needs so barriers are reduced and needs are supported. Is a creative problem-solver who works collaboratively with, and serves as knowledgeable resource and coordinator between client, healthcare team, and available community resources. Assists patients with obtaining community assistance by referral and coordination with appropriate resources. Helps to direct patients/family to appropriate support while maintaining necessary records and data for program growth and development. Builds and maintains relationships with other Parkview navigators as collaborative partners for purposes of improving quality of care. Understands the uniqueness of the individual and seeks the appropriate resources and serves as patient advocate. Provides navigation services and advocates for the team of navigators. Assists with staff interviews, orientation, and training as needs are identified.
Education
Bachelor of Science in Nursing (BSN) required. Masters degree preferred.
Licensure/Certification
RN licensure required.
Experience
3 years of experience in leadership preferred. Demonstrates experience in customer service, establishing priorities, problem solving, and collaborating with internal and external team members.
Other Qualifications
Strong emphasis on interpersonal/customer service skills. Must have good verbal, written, and interpersonal communication skills. Ability and willingness to self-motivate, prioritize, and be willing to change processes to adapt to changing patient, program, or organizational priorities. Knowledge and appreciation of cultural diversity and low literacy issues in care provision. Must be well informed of local programs and support agencies both within and outside the organization.
