Medication Safety Coordinator

Summary

Parkview Health is accepting applications for a medication safety coordinator.

Position Details: 

The Medication Safety Coordinator is responsible for supporting medication quality and safety programs, identifying opportunities to improve the safety of medication-use systems, and assisting in the development of error-prevention strategies targeted to reduce patient safety risks across Parkview Health. Additionally, this role includes ensuring regulatory readiness for the Parkview Health System acute care and hospital outpatient department facilities.

Responsibilities include, but are not limited to:

  • Develop, implement, and assist in the daily functions of the pharmacy medication safety and quality program and initiatives, promoting a strong organizational safety culture.
  • Evaluate medication practices and systems for safety, quality, and effectiveness, including using data analytics tools, such as Excel, to analyze trends and identify improvement opportunities.
  • Identify, investigate, and report medication errors and adverse drug events, leveraging medication safety reporting software and technology to ensure comprehensive documentation and analysis.
  • Participate in, or lead, multidisciplinary teams concerned with medication errors, adverse drug events and reactions, near misses, policy review, safe medication use, new product review, and patient safety to identify risk points and prioritize system improvements to reduce the potential for medication errors and patient harm.
  • Participate in the development, implementation and integration of patient-focused medication quality and safety initiatives in collaboration with other leaders and key stakeholders.
  • Provide education and training on medication safety to front line coworkers
  • Ensure compliance with state and federal medication safety regulatory and legal requirements.
  • Participate in the development and revision of medication policies and procedures.
  • Stay abreast of industry trends, new medications, and regulatory changes related to medication use.
  • Develop and maintain a strong understanding of pharmacy automation and technology systems to identify and mitigate risks, recognizing how automation can both prevent and contribute to medication errors.
  • Precept pharmacy learners in a medication safety learning experience.

This is a full-time, 1.0 FTE position with no weekend or evening staffing requirement. There will be some opportunity for remote work to be agreed upon by the applicant and Pharmacy Director, Quality & Patient Care Services. However, most of the worked hours will be spent at Parkview Regional Medical Center, Parkview Hospital Randallia, or Parkview Business & Technology Center.  Benefits associated with this role include paid time off, annual volunteer time off, and quarterly Compt Lifestyle benefits to be used for personal well-being needs. Parkview Regional Medical Center (PRMC), a member of Parkview Health, is a major regional medical center located in Fort Wayne.  It provides comprehensive medical services to patients in northeastern Indiana, northwestern Ohio, and southern Michigan.  PRMC is a tertiary medical center currently with 544 beds (all private) and is a Level II Adult and Pediatric Trauma Center.  In addition to the medical and surgical populations, it also has specialty centers for stroke, cardiology, orthopedics, and oncology.  The Women’s and Children’s Hospital sits on the PRMC campus and houses a 41-bed Level III NICU, a dedicated and comfortable family birthing center, and pediatrics care.

Parkview Pharmacy has provided advanced pharmacy services in a variety of areas for decades.  Parkview offers a progressive pharmaceutical care environment in a variety of practice settings. Inpatient services include extensive interdisciplinary rounding, graduate medical education engagement, and surgical/critical care and oncology satellite pharmacies.  We have also had tremendous growth in ambulatory pharmaceutical care including primary care, hospital-based clinics, specialty clinics, multiple retail pharmacies, and specialty pharmacy. Position Qualifications:

  • Pharmacy degree from an ACPE-accredited institution
  • Pharmacy licensure or eligibility in Indiana
  • Completion of a PGY1 pharmacy residency with a PGY2 in Medication Safety or equivalent acute care experience.
  • The candidate will need to be comfortable with autonomy, able to self-motivate and function with minimal supervision, skilled in conflict management, willing to actively learn new concepts, and able to deconstruct a process to drive improvement. They will be diligent and able to effectively multi-task. The ideal candidate will also understand the nuances of team dynamics and understand how their words and actions impact others.
