Outpatient PT
Summary
Summary
Provides professional physical therapy services, including evaluation, treatment planning, implementation, and documentation, in accordance with physician orders. Utilizes a variety of therapeutic procedures and assistive devices to help patients achieve their maximum functional potential across all areas of life. Develops individualized treatment plans with measurable and attainable goals, revising them as necessary throughout the course of care. Collaborates with and supervises Physical Therapist Assistants (PTAs), providing guidance on treatment procedures. Educates patients, families, and caregivers on therapeutic exercises and home programs to support continued progress post-discharge. Completes all required documentation and billing in a timely and accurate manner. All care is delivered in alignment with the hospital’s mission, departmental standards, the Indiana State Practice Act, the American Physical Therapy Association, and The Joint Commission.
Education
Must be a graduate of an accredited physical therapy program:

Minimum of a bachelor’s degree for graduates prior to 1/1/2016

Doctorate of Physical Therapy (DPT) required for graduates after 1/1/2016

International candidates must possess a minimum of a bachelor's degree in physical therapy from an accredited institution
-
DPT preferred
Licensure/Certification
-
Must hold or be eligible for licensure as a Physical Therapist in the state where services are provided

Licensure must be obtained within 90 days of hire
-
CPR certification required within 90 days of hire and must be maintained
-
American Physical Therapy Association (APTA) membership preferred
-
Valid driver’s license required if operating a company vehicle; proof of insurance required if driving a personal vehicle
Experience
Prior work experience is preferred but not required, apart from clinical experience gained during an educational program.
Other Qualifications

Must be able to lift up to 50 pounds and assist with lifting up to 100 pounds
-
May be required to push up to 300 pounds
-
Must be willing to travel and work at alternate locations as assigned
