Pharmacist - Home Infusion

Summary

NO weekend shifts (on call every fourth week)
NO holiday shifts (on call during two holidays per year)
8-hour shift between 600am and 630pm, Monday through Friday
Ample support staff
Private desk and cubicle
Flexible Hours within a consistent schedule 
Large patient variety with frequent learning opportunities 
Type III Pharmacy closed to public, serving mostly home-based patients

General Statement of duties:

• Patient outreach and education (medication, infusion equipment)
• Discharge planning coordination with Hospital and Home Health staff
• Care planning and clinical review for I.V., enteral nutrition & hospice patients 
• Multidisciplinary problem solving with clinical and logistical coordination 
• Sterile drug compounding and supervision 
• Drug utilization review (clinical and payer related)

Education: Minimum of a B. S. degree in Pharmacy from an accredited school of Pharmacy. A Pharm. D. degree is preferred.

Licensure/Certification: Must be eligible for or hold a valid and current license as a Registered Pharmacist from the Indiana State Board of Pharmacy. If eligible, must obtain Indiana licensure within 180 days. Additional certifications may be required for specified areas. Must have reliable transportation and provide proof of vehicle insurance. Must have a valid driver's license.

Experience: One to two years home infusion or hospital pharmacist experience preferred. Must have excellent verbal and written communication skills. Must be available to work overtime or flexible hours as needed. Must successfully pass a criminal background check. Must have good verbal and written communication skills. Must be able to lift, push, pull or handle at least 50 pounds.
