Summary
Summary
Provides high-quality patient care through the evaluation, planning, implementation, and documentation of physical therapy treatment. Delivers therapy in accordance with physician orders, utilizing a variety of therapeutic techniques and assistive devices to help patients achieve maximum functional independence. Develops individualized treatment plans with realistic, achievable goals and updates plans as needed throughout the care process. Collaborates with and supervises Physical Therapist Assistants (PTAs), ensuring appropriate delegation of tasks and patient safety. Educates patients, families, and caregivers on therapeutic exercises and home programs to support continued progress after discharge. Completes documentation and billing accurately and on time. All care is delivered in compliance with hospital and rehabilitation department standards, the Indiana State Practice Act, the American Physical Therapy Association (APTA), and The Joint Commission.
Education
Must be a graduate of an accredited physical therapy program with the following degree requirements:

Bachelor’s degree if graduated prior to 1/1/2016

Doctorate of Physical Therapy required if graduated after 1/1/2016

International candidates must possess a minimum of a Bachelor’s degree from an accredited program
Doctorate of Physical Therapy preferred.
Licensure/Certification
-
Must hold or be eligible for licensure as a Physical Therapist in the applicable state; licensure must be obtained within 90 days of employment

CPR certification required within 90 days of employment

Must maintain all certifications and licensure as required

APTA membership preferred

Valid driver’s license required for those assigned a company vehicle; personal vehicle use requires proof of insurance in addition to a valid license
Experience
Work experience beyond clinical education is preferred but not required.
Other Qualifications
-
Must be able to lift up to 50 pounds, assist with lifting up to 100 pounds, and push up to 300 pounds

Strong verbal and written communication skills

Flexibility to work at various locations as needed
