Clinical Pharmacy Specialist

Parkview Health Fort Wayne, IN (Onsite) Full-Time

Parkview Health is accepting applications for a clinical pharmacy specialist in ambulatory care with a focus in population health.

The population health pharmacist will support a growing pharmacy presence within Population Health and our Clinically Integrated Network (CIN). This pharmacist will work as part of a team that includes two existing pharmacists as well as three pharmacy technicians. The ideal candidate must be passionate about engaging with patients, be self-motivated, have excellent time management skills, and be highly flexible. Technology skills, including a strong working knowledge of Excel, and the ability to help build and refine workflows are essential. Dedication to building and maintaining knowledge of Population Health concepts, Parkview’s CIN and value-based contracts, as well as concepts and technical specifications related to quality measures (such as HEDIS, STAR, and ACOs) is critical. The candidate must demonstrate excellent verbal and written communication skills to provide excellent service to all customers, including patients, families, pharmacists, pharmacy technicians, and other health care professionals including system-level leaders.

Essential activities will include:

  • Maintaining an in-depth knowledge and understanding of relevant medication-related measures and their impact on our value-based contracts.
  • Navigating and manipulating reports from insurance companies to support workflows and patient outreach.
  • Completing patient outreach, including telephone calls and electronic communication, to:
    • Assess potential reasons for medication non-adherence and determining a potential root cause for medication non-adherence.
    • Educate patients on various topics, such as strategies to improve medication adherence, the importance of statin therapy, and osteoporosis screening and treatment.
    • Evaluate complex cases identified via consult.
  • Evaluating care gaps related to statin therapy and engaging with patients and providers to develop a pharmacotherapy plan to close statin gaps, whenever possible.
  • Coordinating with other key stakeholders to close care gaps for medication adherence, statin, or osteoporosis measures, when applicable.
  • Identify, develop, and execute opportunities for value-based contracts where pharmacy services could impact cost of care, utilization, or quality measures.

Position Qualifications: Candidates must possess a pharmacy degree from an ACPE-accredited institution and have completed a PGY1 pharmacy residency.  A PGY2 residency in Ambulatory Care is highly preferred. Pharmacy licensure or eligibility in Indiana is required. Candidates must have a high level of clinical practice skills, enthusiasm for teaching, and the interpersonal skills needed to interact effectively with students, health professionals, and patients.  Board certification in Ambulatory care is required within 2 years after hire or as soon as eligible per Board of Pharmacy Specialties requirements.
