Summary
Summary
A leader responsible for providing expertise in the practice of Care Management to produce quality outcomes, promote evidence based practice and demonstrate cost effective practices via collaboration with the patient, personnel, physicians and all applicable stakeholders. Serves as a resource for system-wide and hospital teams/task forces. Provides care to defined patient population and families based on the needs of the individuals according to areas of expertise to promote health, outcomes and improve quality of life. Directly supervises and oversees assigned staff including hiring, training, mentoring, performance appraisals, staff schedules and appropriate disciplinary action in conjunction with the PPG Care Coordination Manager.
Education
Bachelor of Science in Nursing (BSN) required.
Licensure/Certification
Must be an RN with current Indiana licensure.
Experience
5 years of leadership and care management experience preferably in Ambulatory Care, Community Public Health, Case Management, Ambulatory Clinic Management and Coordinating Care across multiple settings and with multiple providers.
Other Qualifications
Must be able to complete job duties autonomously. Demonstrated leadership skills and strong organizational skills. Broad knowledge base of care coordination. Ability to interpret and analyze information. Exceptional communication skills, both written and oral, ability to positively influence others with respect and compassion; fluency in a foreign language is a plus. Requires ability to communicate and resolve conflict with physicians, other health care providers, families and patients with diverse opinions, values and religious/cultural ideas. Strong work ethic built on a foundation of proactivity and team work. Ability to navigate ambiguity with the aid of structured problem solving techniques.
