Outpatient PT

Summary

Summary
Provides professional patient care by evaluating, planning, implementing, and documenting physical therapy treatment. Delivers therapy in accordance with physician orders, using a variety of therapeutic techniques and assistive devices to enhance functional abilities across all areas of a patient’s life. Develops individualized treatment plans with measurable, achievable goals based on patient needs, and updates them as necessary throughout the course of care. Supervises and collaborates with Physical Therapist Assistants (PTAs), ensuring appropriate delegation and oversight of treatment. Educates patients, families, and caregivers on home exercise programs to continue progress post-discharge. Completes all required documentation and billing in a timely and accurate manner. Care is provided in alignment with the philosophy of the hospital, rehabilitation department standards, the Indiana State Practice Act, the American Physical Therapy Association (APTA), and The Joint Commission.

Education

  • Graduate of an accredited physical therapy program.

  • Degree requirements:

    • Bachelor’s degree (if graduated prior to 1/1/2016).

    • Doctor of Physical Therapy (DPT) required (if graduated after 1/1/2016).

    • International candidates must have a minimum of a Bachelor’s degree in Physical Therapy from an accredited institution.

  • DPT preferred.

Licensure/Certification

  • Must hold or be eligible for licensure as a Physical Therapist in the state where services are provided. Licensure must be obtained within 90 days of employment.

  • CPR certification required within the first 90 days of employment.

  • Must maintain all licenses and certifications as required.

  • APTA membership preferred.

  • If assigned to drive a company vehicle, a valid driver’s license is required. For personal vehicle use, proof of insurance and a valid license are required.

Experience
Prior work experience preferred but not required beyond clinical experience gained through an accredited educational program.

Other Qualifications

  • Must be able to lift up to 50 pounds, assist with 100 pounds, and may need to push/pull up to 300 pounds.

  • Flexibility to work at alternate locations as assigned.
