PPG Rounding LPN
Summary
Responsible for rounding on patients in the hospital, documentation, review of hospital ordered labs and x-rays and communicating back with physicians status and concerns. Works with management and physicians in a collaborative manner, assuring all policies, procedures and programs are compatible with goals and objectives of line of business.
Collaborate with physicians and other departments to achieve quality and safety goals. Triage clinical concerns and communicate status on labs and imaging back to physician. Demonstrate commitment to service with a passionate desire to improve the patient experience. Handle all aspects of inpatient hospital discharge: post-discharge follow-up appointments, medication prior authorizations and outpatient phone calls for any clinical concerns after discharge. Participate in daily huddles. Collaborate with nurse case managers on ECF placement. Pend orders in EMR for physician sign off
Education
Must be a graduate of a Licensed Practical Nursing (LPN) program.
Licensure/Certification
Active LPN license and CPR required.
Experience
Required: Solid knowledge base of surgical procedures. 1-2 years experience in a physician office setting or related field. Preferred: Electronic Medical Record experience preferred.
Other Qualifications
Must have excellent interpersonal skills and represent our organization in a positive manner. Must have solid verbal and written communication skills. Knowledge of clinical operations, service excellence, clinical quality and education initiatives. Must be familiar with computers.
