Lung Nodule Pm Nurse Navigator
Summary
Summary
Under the direction of the Lung Nodule Program Manager, the Lung Nodule Program Nurse Navigator serves as a critical safety net within the health system to ensure that patients with lung nodules are not lost to follow-up and that care decisions are made in a timely and coordinated manner. Working behind the scenes, facilitates communication among providers, advances the decision-making process, and supports timely diagnostic and treatment interventions. Collaborates closely with the Lung Nodule Program Advanced Practice Providers and Pulmonologists to initiate and coordinate appropriate diagnostic testing, and directly contacts patients identified with a lung nodule who do not have an established provider to ensure they receive clear communication about recommended next steps in their care. Manages complex AI enabled dashboards that track a high volume of patients, as well as read and interpret imaging scans and reports applying evidenced-based care pathways in alignment with NCCN, Fleishner, or Lung Rad guidelines. At all times, serves as an advocate for the patient to provide education and information relative to the testing and decision making that may need to occur to move the patient through the continuum of care.
Education
Bachelor of Science in Nursing (BSN) required. MSN preferred.
Licensure/Certification
Current state license as an RN required.
Experience
Minimum of 5 years of clinical experience in Pulmonology, Oncology, Thoracic Surgery, or related specialty. Minimum of 5 years working closely with physicians/clinical providers.
Other Qualifications
Demonstrated expertise in clinical reasoning, critical thinking, and evidence-based decision making. Exceptional organizational, communication, and problem-solving skills. Demonstrable ability to manage information/data through the use of technology. Must be detail oriented, and able to handle multiple tasks with a high level of accuracy. Must possess exceptional communication and collaboration skills. Lifelong learner willing to engage with continually changing technology.
